What the visit request form collects

When you choose Send visit request, we save what you entered: your name, email, optional phone, the setting, an organization or facility name (care communities and healthcare settings only), city, approximate timing, whether you are interested in one or recurring visits, and your optional message. We also record when the request was received and a random reference number.

We use this information to respond to your request and discuss a possible visit. Submitting does not sign you up for marketing. We do not ask for street addresses, payment details, diagnoses or medical records.

Please do not include patient, resident, or child names, diagnoses, or medical records.

Visit outlines

The planning tool holds only preset choices in browser memory while the page stays open, and resets when you refresh. Nothing from it is saved unless you send a visit request with the outline attached; then the preset choices and a generated outline are saved with your request. You can remove the outline before sending. Copy, print and Email this plan work only when you choose them; email is sent from your own email application.

Who can see requests

Saved requests are visible only to authorized Sound of Care staff who sign in to a private staff area. Staff may add private follow-up notes and status updates. Requests are stored with our website’s hosting and database provider, which processes them on our behalf under its own terms.

Artist & performer applications

If you apply through the Artists & Performers form, we collect the contact, profile and proposed-experience details you enter so we can review your participation. Applications are not published automatically. If we later approve a profile or offering, only the public profile content we review with you may be published, after a separate review. Contact details and our review notes stay private. Authorized Sound of Care staff can access applications.

Sign-in and browser storage

Visitors do not need an account. Staff sign-in keeps a session in that staff member’s browser storage until they sign out or it expires. The public form and planner do not store your details in browser storage, cookies or the page address.

Website hosting

Hosting and delivery providers may process technical request information, such as network addresses, browser details and access logs. To limit repeated or automated submissions, the form uses short-lived, one-way hashed connection identifiers rather than storing raw network addresses. The site includes error-reporting hooks that may share page paths and error details with the hosting or editor environment when available.

We have not added an advertising or visitor-analytics service. Provider policies and technical handling may differ; this page does not promise a specific retention period.
